Our aim is to Create a safe space for recovery

Home From Hospital Care is a registered charity based in the West Midlands. Since 1986, we have worked to ensure that no one feels alone, unsupported, or unsafe when returning home from hospital or facing challenges in daily living.

Our work has always focused on practical assistance, emotional reassurance, and meaningful connection. This has helped us win the The Queen’s Award for Voluntary Service.

We are committed to reducing isolation, promoting independence, and preventing avoidable hospital readmissions by ensuring people receive the right support at the right time.

A community where every individual can return home safely, live independently with dignity, and feel connected rather than isolated — regardless of age, circumstance, or vulnerability.

We envision a future where community-based support is recognised as an essential part of recovery and wellbeing.

For over 35 years, we have supported adults across the West Midlands to return home safely, reduce isolation, and maintain independence.

Through practical and emotional support, we have helped prevent unnecessary hospital admissions, strengthened partnerships with health and community organisations, and improved wellbeing for vulnerable residents.
